INSTRUCTIONS ON ACR
Please find the instructions on the ACR below. They are comprehensive and useful
(A).HU Doctors/OPD Doctors of CH/Divl. Hospital
1. Reduction in number of sick certificates issued, man-day’s loss, imprest expenditure in percentage against corresponding period of last year.
2. No. of multipurpose camps, Health check up camps, Immunization camp, School health camps, IUCD insertion camps, Special cleanliness drives. Field action group meeting. Health education meetings on public health and alcoholism/AIDS/Hepatitis-B/TB in society with out put there of held against last year.
3. Innovation works undertaken, e.g. tree plantation, gardening, improvement in methods of cleanliness maintenance, renovation work, achievements in infection control/BMW mgt works, quick disposal methods of patients.
4. Condemnation/replacement of equipments and furniture etc. staff training.
5. Enhancement in PFA act/food quality control activity, potable water supply monitoring, family planning anti malarial and immunization work indicating comparative achievements against last year in percentage/number.
6. Inspections of station and updating of ARME van/POMKA.
7. Time bound clearance of PME cases etc.
B. Specialists/ Indoor Doctors/ etc.
1. Scientific papers presented/published & conference attended and feed back presentation given in hospital plus all items under A.3 above.
2. Innovative works – good bed head tickets, treatment protocols made for work. Measures taken for good record keeping/accountal in stores, house keeping infection control and BMW mgt. laboratory investigation reporting system, computerization in work place., full scale use of laptop in professional administrative works.
3. Research on particular disease/activity with outcome thereof.
4. New machines procurement and installation and utilization, AMC of equipments, condemnation and replacement of furniture and equipments.
5. To bring down ALS and to improve BOR.
6. Enhancement in Endoscopic examination, surgical performance, pace maker implantation with success rates, TMT. Echocardiography/USG/Colour Doppler done. New procedures_ introduced with comparative data.
7. DNB trainees of number of classes/seminars,clinics, time bound six monthly appraisal and passing rates.
8. Motivation of staff and training imparted to staff with patient satisfaction rate determined by random feed back system and comparative data.
9. “NIL COMPLAINT OBJECTIVES” on patient care in true and humanitarian sense.
10. reduction inmaternal/foetal or neonatal deaths, deliveries against last year.
11. Upliftment in investigation facilities/procedures, blood bank services/physiotherapy unit etc.
C. Administrators (MD,CSS,CMS & MSs, Sr.DMO/Admn. & H&FW) and Sr.DMO/Stores.
1. Clearance of diet bills, audit reports condemnation and disposal of stores in terms of quantum, time bound tender finalization without extension of any contract, timely uniform supply, filling up of vacancies and selection/promotions etc.
2. Achievements in M&P and works programme, AMCs done, time bound DNB/House surgeons appointment and their full utilization, doctor & staff training, infection control/BMW mgt. disaster mgt. training and tie up with private hospital with financial benefits/implication thereof, medical boards clearance etc.
3. Innovations and improved house keeping – in kitchen, OT/OPD/IPD/investigation/RELHS services, record keeping, ward/office/investigation deptts.
4. Favourable financial implication on utilization of visiting specialists.
5. H&FW – qualitative and quantitative improvement in sanitation, septic tanks/drains dislodging /desilting/cleaning, anti malarial/PFA Act quality control of food works done. Plus all subjects under A.2 above.
6. Action on absenteeism of staff with development in counselling_/DAR action thereof.
7. Detection of _short expiry dated medicines and time distribution to other unit., innovating methods in accountable/storage/distribution system, immediate disposal of condemned medicines stores items, timely submission of annual indents and supplementary indents thereby medicine LP and any other innovative works.
